Perimenopause

Perimenopause is the transitional phase leading up to menopause, typically beginning in a woman’s early- to mid-40s. During this time, ovarian function becomes erratic, resulting in significant and often unpredictable fluctuations of estrogen and progesterone rather than a steady decline. Mood changes can be common, as these reproductive hormones play a crucial role in regulating neurotransmitters like serotonin and dopamine. While perimenopause is a natural biological process, the physiological shifts involved can feel disorienting and intense, lasting anywhere from a few years to over a decade before the cessation of menstruation.

The symptoms of perimenopause are diverse. While physical vasomotor symptoms like hot flashes and night sweats are the most widely recognized, the neuropsychiatric effects can be even more distressing. Many women experience profound changes in their mental health, including heightened anxiety, sudden irritability or rage, depressive episodes, and cognitive difficulties often described as "brain fog." Sleep is frequently disrupted, leading to chronic insomnia that further exacerbates mood instability. It is vital to understand that these symptoms are not "all in your head" or a failure of coping; they are direct consequences of how hormonal volatility impacts brain chemistry.

We begin with a comprehensive evaluation to distinguish hormonal mood shifts from other psychiatric conditions. Management often includes the use of targeted medications; this may involve SSRIs or SNRIs, which are non-hormonal options proven to treat mood symptoms effectively while also reducing hot flashes. Hormonal Replacement Therapy (HRT), where clinically appropriate, can be another compelling option to consider in collaboration with your gynecologist or endocrinologist.
